At Animal Vaccination Clinics™, we provide affordable preventive veterinary medicine to the community. Vaccines are provided to Healthy Animals, owner’s consent that their pets/animals was not exposed to animal bites, bats or sick animals within the past 10 days.
I accept that the veterinary health care professionals administering vaccines or other treatments to my pet/animal has determined my pet to be healthy enough to receive these treatments, but further diagnostic measures such as laboratory tests (blood, urine, fecal, etc.) , radiographs, ultrasonography may be necessary to completely assess my pet’s health and I accept the current assessment consult and I accept the current assessment without these other tests as adequate measures for the administrations of biologicals (vaccines) and or parasiticides.
I, also, acknowledge that vaccines or medication reactions, although rare, may occur, and agree to seek prompt medical attention for my pet, at my own expenses, at the nearest open licensed veterinary facility, should such a reaction occur.
I understand that I am solely responsible for keeping track of and bringing my pet animals for flow up vaccines and that today’s vaccinations/treatments do not include the cost of any future vaccines/treatment. (❑Your ID is required for Veterinary Health records).
Acceptance of Terms: By accessing or using our services, you agree to be bound by these Terms and Conditions. If you do not agree with any part of these terms, you may not access or use our services.
Use of Services: Our services are provided for informational and educational purposes only. You may not use our services for any illegal or unauthorized purpose.
